Transition of ventilator-assisted individuals from the ICU to home: sub-analyses
Summary
The Transitional Home Ventilation Program was established at the West Park Healthcare Centre in 2010 to monitor ICUs and facilitate transition through the program to an appropriate community or long-term care setting, rather than remaining in the ICU. The purpose of this request is to understand the associated costs of health care use after discharge from the program to elucidate potential cost savings to stakeholders.
