Summary: The SWRCP seeks to understand the regional system baseline measures in terms of provider capacity and burden of disease in order to identify gaps and opportunities for quality improvement. The request details patters of diagnoses, referral and treatment from 2010 to present. Results will provide a baseline assessment and assist in identifying opportunities to increase system capacity and better determine appropriateness of care by the different providers.
