Patient and health service utilization characteristics of the 65+ population visiting the UHN Eds
Summary
The main objective of this project is for the purpose of Planning. The SEMC is developing new and improved care processes for complex medical and supportive needs for frail seniors in the emergency department. Frail seniors in the ED require unique processes and care that caters to critical issues for older adults. The results of this research question will directly impact the care provided by the seniors-focused multidisciplinary team including nursing, social work, and occupational therapy integrated with current ED teams at TGH and TWH and the purpose-built space opening in January 2026. We hope to translate our learnings internationally and influence system-wide change.
