Summary: Durham Region requests measures of the health status of its residents by neighbourhood and over time by examining the prevalence and incidence of diabetes, asthma, chronic obstructive pulmonary disease and hypertension. The information will be used to assess the health status of vulnerable and special needs populations, specifically those living with mental illness and addiction, for the purposes of population health assessment and future program planning and evaluation.
