Summary
The London Health Sciences Centre explored the health care utilization pathways of individuals who accessed and were discharged from the Clinical Teaching Unit, an internal medicine ward at LHSC, over the course of one year. Research evidence will be used to aid in the development of strategies to improve the quality of care and support those who are high users of the health system and at high risk for readmission.
