Summary
Heart function clinics are established to reduce mortality, hospital admissions and emergency room visits and to improve the quality of life for patients with heart failure. The purpose of this request is to determine whether a single-centre, community hospital–based clinic led by a nurse practitioner resulted in reduced health care use and influenced outcomes.
