Summary: A series of analytical projects is being sought by the Algoma OHT in order to inform the planning and delivery of health, social and health promotion services regionally. To ensure that no one gets left behind, the AOHT is seeking to take a population health planning approach by identifying patterns of health and care, including focusing on conditions and their associated factors of influence. Specifically, for the priority population on Complex Chronic Disease, the AOHT has information on incidence and has been able to identify high users of the Emergency Department, however, needs access to system-wide data to inform program development, including priority setting for target populations and interventions in the community.
