Ann Young

Dr. Ann Young is an assistant professor and clinician investigator in the Division of Nephrology, Department of Medicine at the University of Toronto and St. Michael’s Hospital, Unity Health Toronto, and an adjunct scientist in the Kidney, Dialysis and Transplantation Research Program at ICES Western. She received her MD and completed her internal medicine residency and nephrology fellowship at the University of Toronto and holds a PhD in clinical epidemiology and health services research from Western University.

Dr. Young’s research program focuses on transforming the delivery of care for people living with chronic kidney disease (CKD). She is particularly interested in leveraging linked population-based health data to identify unmet healthcare needs and develop and evaluate proactive models of care that connect people with appropriate services earlier in their disease trajectory. Central to this work is KidneyCare Outreach, a novel population-based initiative that uses routinely collected health data to identify individuals at high risk of kidney failure who are not receiving guideline-concordant nephrology care and proactively connect them with appropriate services. This work is establishing a new approach to using population-based health data not only to evaluate health system performance, but also to identify care gaps and intervene to improve patient outcomes.

Dr. Young is supported by a KRESCENT New Investigator Award and serves on the Ontario Renal Network Early CKD Priority Panel and the Canadian Blood Services Kidney Transplant Advisory Committee. She is also Co-Director of the Kidney Transplant Fellowship Program at St. Michael’s Hospital.
