Health outcomes of a cardiovascular rehabilitation and secondary prevention services program (CRSP) in the Central East LHIN
Summary
In an effort to redesign the CRSP system of care, the Regional Cardiac Care Program in the Central East LHIN developed a new delivery model that provided standardized care and high-quality services and has received increased funding and capacity with every year. Research evidence gathered from this request will be used to evaluate this model of care delivery and determine program effectiveness and support policy development and planning that aims to further reduce health care costs.
