Summary: The Hamilton Public Health Services (HPHS) is supporting the Primary Care Network with building a Geographic Information System based platform to produce a one-stop, interactive planning tool for primary care access and attachment. One of the inputs will be the demographic and clinical characteristics of people in the different attachment profiles.
