Summary
The knowledge users requests information on comparing two definitions used to diagnose and treat heart failure. This is part of the Heart Failure Care Initiative championed by CorHealth and implemented first by the London Health Sciences Centre. The findings will help to organize and integrate the care of those living with heart failure, as well as inform future system planning for this cohort.
