Evaluating Reconnect’s ACT, ADP, Transitional Care, and Assisted Living Services Program
Summary
Reconnect community health services offer specific mental health and seniors’ care services. The knowledge users are interested in understanding whether enrollment into their services reduces the rates of hospital admissions and/or re-admissions, comparing a 12-month period prior to and following enrollment in ACT, ADP, transitional care, and Assisted Living Services. This project will inform decisions for program improvement.
