An appraisal of grassroots Patient Medical Home practice developments in Canada: a College of Family Physicians of Canada commissioned study
Summary
The project will measure the impact that identified, patient medical home-aligned primary care practices in Ontario have on access and delivery of comprehensive care. Health administrative and billing data will be analyzed for outcomes like the number of annual patient visits, or the proportion of patients with access to preventative care.
