Summary
META:PHI Toronto is a collaboration between several Toronto Central LHIN hospitals to implement an integrated care pathway for patients with addiction. The purpose of this request is to facilitate the implementation of evidence-based addiction treatment protocols in different health care settings in order to improve system integration and increase access to quality addiction care throughout the city.
