Summary: The Knowledge User is seeking better data of its attributed populations in order to address health outcomes within the Guelph and Area Ontario Health Team (G&A OHT). Using attribution methodology developed by the Ministry of Health, the evidence collected will be used to develop Integrated Primary Care Teams (IPCTs), a model which integrates home and community care into primary care, across the G&A OHT. Results of the analyses will be used to inform decision makers about (re-)allocation and integration opportunities to support the development of the IPCT model in the G&A OHT.
