The new model will be associated with increased distress screening rates, patient satisfaction, uptake of PSO care in demographically vulnerable groups and decreased healthcare utilization. The data required from ICES will include ER visits and hospital admissions for patients at PMCC before and after implementation of routine emotional distress screening and before and after implementation of Proactive PSO.
